Infertility - Diagnosis and Treatment

In Vitro Fertilization

IVF involves the stimulation of follicles with gonadotropins combined with ultrasound monitoring.  HCG or Lupron is used to prepare the oocytes for retrieval.  Retrieval, or egg collection, is performed by trans vaginal needle aspiration before the oocytes are released into the abdomen.  The oocytes are then combined with the sperm so that fertilization may occur.  Techniques such as ICSI and assisted hatching are additional procedures that can be performed during the IVF process.  Patients with tubal factor infertility or male factor infertility will need IVF if the abnormalities are severe enough to preclude insemination cycles.  Also, patients that have failed other therapies such as clomid, IUI, or COH may be referred for IVF.

IVF - Sperm Injection and Male Infertility

Preimplantation Genetic Screening (PGS)

Preimplantation genetic screening is an advanced reproductive technology in which a single cell, or blastomere, is removed from the embryo and evaluated by genetic testing for various reasons.  The most common reason is for the prevention of genetic diseases, which may be present in the embryo either from spontaneous mutations or from disorders inherited from the parents.  The procedure involves making a small hole in the outer shell, or zona, using an acid solution or a laser with the assistance of a specialized microscope.  A small micromanipulation pipette is then used to gently pull cells, which can be used for genetic testing.

Frozen Embryo Transfer - FET

A frozen embryo transfer cycle is a procedure using cryopreserved embryos from a previous IVF treatment.  The frozen embryos have a slightly lower chance of resulting in a pregnancy than a fresh embryo transfer done in conjunction with IVF.  The FET does offer a much easier approach to attempting pregnancy than another IVF cycle and represents an excellent option for continued treatments.  Embryos frozen from previous IVF do not have a shelf life and can be used years after the initial IVF cycle from which they were produced.  Couples having a successful IVF treatment can return several years later and have an FET cycle done for far less cost than IVF
